Abstract

|
목적: 본 연구의 목적은 COVID-19 환자를 간호한 간호사의 외상 후 스트레스, 직무스트레스, 피로 및 사회적 지지 정도를 파악하고 외상 후 스트레스에 미치는 영향요인을 확인하기 위함이다.
연구방법: 연구대상은 3개 종합병원에 근무하는 간호사 150명이었으며 자료수집기간은 2020년 9월 11일부터 2020년 9월 21일까지였다. 자료 분석 방법은 SPSS WIN 25.0 통계프로그램을 이용하였다.
연구결과: 연구 대상자의 외상 후 스트레스는 총점 88점 만점에 22.27±15.49점 이었으며 외상 후 스트레스 고위험군은 35.3%, 외상 후 스트레스 위험군은 20.0%, 정상군은 44.7%이었다. 본 연구 대상자의 일반적 특성 및 직무관련 특성에 따른 외상 후 스트레스는 자가격리경험(t=2.15, p=.033), 환자관리매뉴얼제공(t=-2.40, p=.026)에서 통계적으로 유의한 차이가 있었다. 본 연구 대상자의 외상 후 스트레스는 직무스트레스(r=.48, p<.001), 피로(r=.58, p<.001)와는 정적상관이 있었으며, 사회적 지지(r=-.22, p=.005)와는 부적 상관관계를 보이는 것으로 나타났다. 직무스트레스와 피로 간에는 높은 정적상관(r=.74, p<.001)이 있었으며, 직무스트레스와 사회적 지지(r=-.17, p=.030), 피로와 사회적 지지(r=-.17, p=.029) 간에는 부적 상관관계를 보이는 것으로 나타났다. 본 연구 대상자의 외상 후 스트레스에 영향을 미치는 변인은 피로(β=.56, p<.001), 환자관리 매뉴얼제공(β=-.24, p<.001), 자가격리경험(β=.18, p=.006) 이었다. 변인들에 의한 설명력은 42.0%이었다.
결론: 피로, 환자관리 매뉴얼제공, 자가격리경험은 외상 후 스트레스에 영향을 미치는 요인으로 확인되었다. 따라서 신종감염병 환자 간호에 참여한 간호사의 외상 후 스트레스를 예방하기 위하여 간호사의 피로를 관리하고 표준화된 감염병 환자 관리 매뉴얼 제공과 자가격리경험이 있는 간호사를 위한 심리적 중재 프로그램이 마련되어야 할 것이다. 본 연구를 바탕으로 신종감염병 환자를 간호한 간호사의 외상 후 스트레스를 예방하기 위하여 피로를 감소시키기 위한 적절한 간호 중재가 선행되어야 하며 자가격리경험이 있는 간호사는 외상 후 스트레스 고위험군에 속할 가능성이 높으므로 적극적인 관리가 필요할 것이다.

Purpose: The purpose of this study was to investigate post-traumatic stress, job stress, fatigue, and social support of nurses in direct care for COVID-19 patients and to identify the factors affecting post-traumatic stress among the nurses.
Methods: The participants were 150 nurses from three hospitals. Data were collected from September 11, 2020 to September 21, 2020. The data were analyzed with SPSS/WIN/25.0 program.
Results: The mean score of post-traumatic stress was 22.27±15.49 (range 0~88). The percentage of high risk group of post-traumatic stress was 35.3%, risk group was 20.0%, normal group was 44.7%. Post-traumatic stress showed statistically significant differences according to quarantined experience (t=2.15, p=.033), and provision of COVID-19 manual (t=-2.40, p=.026). Post-traumatic stress was positively correlated with job stress (r=.48, p<.001), and fatigue (r=.58, p<.001), and it was negatively correlated with social support (r=-.22, p=.005). Job stress was positively correlated with fatigue (r=.74, p<.001), and it was negatively correlated with social support (r=-.17, p=.030). Fatigue and social support (r=-.17, p=.029) had a negative correlation. The results of regression analysis showed that the factors affecting post-traumatic stress included fatigue (β=.56, p<.001), provision of COVID-19 manual (β=-.24, p<.001), and quarantined xperience (β=.18, p=.016) and that the total explanatory power was 42.0%.
Conclusion: In this study, fatigue, provision of COVID-19 manual, and quarantined experience were found as influential factors of post-traumatic stress among nurses in direct care for COVID-19 patients. Therefore, development of the intervention for reducing the fatigue should consider to prevent post-traumatic stress in nurses. Also, provision of COVID-19 manual for nurses and psychological intervention program for nurses experienced quarantine are necessary.
